Ward Activity Pages

Please complete ALL permission forms by May 12, 2026.

ALL FORMS ARE REQUIRED TO ATTEND FOR EACH YOUNG WOMEN AND LEADER.

Step 1 – Permission & Medical Release Form

All participants and leaders must submit a release form to the ward camp director.

Please make sure BOTH a guardian/parent and the participant (camper) sign the bottom. 

After using a pdf filler to fill out and sign (both parent and camper),  you can email it (to littletonstakeywcamp@gmail.com) or print it and give it to your ward camp director, Sami Thompson, Heather Shafer,  or Dia Sabey.

Step 3 – Insurance (For Drivers Only)

For all Leaders driving any young women, they must send a picture of their auto insurance card to
